Once an air transfer is decided, the first question families face is usually this: charter a dedicated medical aircraft, or book a stretcher position on a commercial flight? The cost difference can run to several times — yet "expensive" does not mean "necessary," and "cheap" does not mean "feasible." The right choice depends on balancing four dimensions: medical condition, transfer distance, time requirements, and budget. This article breaks each one down.

1. Medical Condition: The Hardest Filter

The patient's condition is the first sieve. Critically ill patients requiring continuous invasive ventilation, ECMO support, or hemodynamic stabilization can only travel by medical charter — the cabin environment and power supply of commercial aircraft cannot support ICU-grade equipment, and airlines will not approve boarding for such patients. Patients who are relatively stable and require only low-flow oxygen or intravenous medication may be candidates for a commercial stretcher position. Patients in the middle ground — for example, postoperative recovery cases or those needing non-invasive ventilation — require case-by-case evaluation by a medical consultant.

2. Distance: Where Stretcher Positions Make Economic Sense

Distance directly affects cost-effectiveness. Commercial stretcher positions are priced by "seats occupied plus medical surcharge"; on short-haul routes (under three hours), the cost is typically one-fifth to one-third of a medical charter — a clear economic advantage. As the route lengthens, however, the number of seats a stretcher occupies increases (long-haul widebodies usually require six to nine seats), and once accompanying medical staff fares are added, the gap with a charter narrows. For long-haul cross-border transfers, a stretcher position remains a high-value option when the patient's condition permits — but if connections are involved, repeated handling increases risk, making a direct charter the safer choice.

3. Time Requirements: The Charter's Core Advantage

The medical charter's greatest advantage is schedule autonomy: apply anytime, depart anytime, fly direct — from decision to takeoff in as little as 12 hours. Commercial flights must conform to existing timetables; stretcher installation requires 48–72 hours of advance application with the airline, and not every flight is stretcher-capable — during peak periods, no suitable service may be available for days. For patients whose condition has a window of potential deterioration, "waiting for a flight" is itself an unacceptable risk, and a charter becomes the only option.

4. Budget: Do the Math Properly

On cost, domestic medical charters typically range from the low hundreds of thousands to several hundred thousand RMB depending on aircraft type and distance, while domestic commercial stretcher positions generally run in the tens of thousands. One caution: compare "all-in" prices. Charter quotes usually include the medical team, equipment, and ground transfer; a stretcher plan must add costs for accompanying medical staff, equipment rental, and ambulances at both ends. In addition, some high-end medical insurance policies reimburse the two options at different rates — confirm with your insurer before deciding.

5. Decision Reference: A Simple Guide

In brief: critical or unstable condition → medical charter (no alternative). Mild-to-moderate and stable, short-haul, no time pressure → commercial stretcher preferred. Mild-to-moderate but long-haul or cross-border with urgency → medical charter is safer. Anything in between → defer to a professional medical consultant's evaluation. Whichever option is chosen, medical assessment must come first — establish "can they fly" before discussing "how they fly."
